Healthy Families Network (HFN)
HFN is a consortium lead by Metro Health’s Healthy Start to strengthen community partnerships and linkages to reduce infant mortality and support successful birth outcomes in Bexar County.
Goals
- Create a sustained community network of invested partners.
- Conduct PPOR Analyses and other data collection and analyses activities to support and inform program and community interventions.
- Increase community capacity to address infant mortality and low-birth weight.
- Minimize barriers and gaps in structures that do not support or promote positive birth outcomes.
Meetings
Day: 2nd Monday of each month
Time: 11:30 a.m. - 1:30 p.m. (lunch provided)
Location: Meeting locations alternate every month. Call 210-207-4725 to confirm the location.
Work Group Activities
Work Group 1: Data, Resources, Utilization and Management (DRUM)
- Conduct data analysis of fetal and infant deaths in Bexar County.
- Set intervention priorities for the community based on risk factors and causes of fetal and infant deaths.
Work Group 2: Reproductive Life Planning (RLP)
- Develop a pilot intervention to expand the capacity of community agencies.
- Establish the original membership, vision, mission and goals of HFN.
Work Group 3: Healthy Outcomes Through Perinatal Education and Support (HOPES)
- Serve as an advisory board for several Parents As Teachers home visiting programs.
- Work on initiatives to improve home visiting in Bexar County.
Work Group 4: Cultural Awareness, Respect and Empowerment (CARE)
- Ensure HFN activities and products are culturally-sensitive and useful to communities.
- Implement the use of the culturally and linguistically appropriate services (CLAS) standards.
Committees
Standing and ad-hoc committees provide oversight and communication between work groups.
- Steering Committee
- Advisory committee, consisting of chairs and co-chairs of each work group and guides HFN toward achieving its goals.
- Baby Buggy Walk in the Park Event Planning
- The group meets throughout the year to plan the activities surrounding the yearly infant mortality awareness month observance in October.

Agencies/Organizations committed to HFN may sign a Memorandum of Understanding, which allows your logo to be added to HFN products.
